Terms and conditions
By entering the Oklahoma State University “Poke-a-thon” Contest you agree to be bound by these Official Rules:
Eligibility
This contest is open to OSU-Stillwater students actively enrolled for fall 2021 who are fully vaccinated. Those not fully vaccinated at the beginning of the contest may enter at any time once they are fully vaccinated.
Individuals who cannot be vaccinated due to medical conditions or sincere religious objections may fill out the exemption/declination form and also be entered to win prizes. Students must upload the completed form to their Patient Portal following the same instructions for uploading vaccination proof.
Definitions
Fully Vaccinated: To be considered fully vaccinated, a student must receive two doses of the Pfizer or Moderna vaccines, one dose of the Johnson and Johnson vaccine, or receive a World Health Organization approved vaccine.
Students: Actively enrolled students for fall 2021, excluding concurrently enrolled high school students and full-time OSU employees enrolled in courses. VetMed students are eligible; however, students of other institutions attending OSU solely for clinical rotations are not eligible for the grand prize but are eligible for lesser prizes.
Contest dates
The contest begins July 29, 2021 at 2 p.m. CST and ends Oct. 13, 2021, at 5 p.m. CST. Entries received after 5 p.m. Oct. 13, 2021 CST will not be considered for awards, regardless of whether the late receipt resulted from human, computer or transmission error. For individuals receiving the two-dose Pfizer or Moderna vaccine, both vaccine doses must be administered by the 5 p.m. Oct. 13 deadline.
Entry guidelines
Entry Process: The contest is open to fully vaccinated, current OSU-Stillwater students actively enrolled for fall 2021.
Students fully vaccinated at University Health Services are automatically entered.
Students fully vaccinated at other health care providers or at vaccine clinics may upload a picture of their vaccine card to their UHS portal to be entered into the prize drawings.
Students who have not been vaccinated can make vaccine appointments at UHS or stop by a pop-up OSU vaccine clinic. Once they are fully vaccinated, they will automatically be entered to win.
Students who have not been vaccinated but become fully vaccinated through an off-campus provider can upload a copy of their vaccination card to their UHS portal.
Prize drawings
Winning names will be drawn by UHS staff at regular intervals determined by the University. The winning students will be contacted by Students Affairs once eligibility has been verified.

*If a student wins any prize other than the grand prize, the student remains eligible for the grand prize drawing but will not be eligible for any other prize drawings.
Notification of winners
Winners will be contacted by Student Affairs once eligibility has been verified. Their name and prize will be announced publicly on OSU social media channels. If a winner does not respond to notification of the win within 14 days, the winner will be disqualified and a new winner will be chosen, based on the established judging criteria. Any scholarship awarded is subject to all applicable federal and state laws and may be subject to reduction based upon applicable scholarship or financial aid limits.

Use of submissions
By entering this contest, the student hereby grants to Oklahoma State University and its affiliates* the right to take their picture and to reproduce, use, exhibit, display, broadcast, distribute, exploit, modify, adapt, and create derivative works of photographs, videotaped images or video/audio recordings of me (“Materials”) by incorporating them into publications, catalogues, brochures, books, magazines, photo exhibits, motion picture films, videos, electronic media, web sites, and/or other media, or commercial, informational, educational, advertising, or promotional materials or publications related thereto. Oklahoma State University reserves the right to modify or change the Poke-a-thon contest rules at any time. Contest is subject to all applicable United States federal, state, and local laws and regulations.
Contest ownership
Oklahoma State University and its affiliates are the sole owners and operators of the Poke-a-thon contest. Official rules, terms and eligibility requirements, prize information and selection are administered by the University. By participating, entrants release Facebook of any ownership. The promotion is in no way sponsored, endorsed or administered by, or associated with Facebook.
*Affiliates include the Division of Enrollment and Brand Management, Athletics, university departments, academic colleges or programs.
Waiver of liability
By hosting this contest, OSU in no way endorses or sponsors any manufacturer of vaccinations. By entering this contest, the student is choosing to voluntarily receive the vaccination(s) and assumes all liability therefore. OSU is not responsible for any injuries or illnesses occurring as a result of a student obtaining vaccinations and entering this contest.
